Feliz Martone, JD will help you explore the practical side to end of life planning. You'll receive education from a licensed attorney to help you protect yourself, your assets and your loved ones. You will be able to leave this workshop with your basic end of life care planning documents completed and ready to be filed with your hospital and physician's offices.
Notary will be available on-site to notarize your Advance Healthcare Directive, Durable Power of Attorney for Healthcare, Durable Power of Attorney for Financial Power of Attorney and a HIPPA Authorization.
